4th Note bugged?

Hello. I am currently trying to find all the hidden notes and it seems that my 4th note never spawned.

It's supposed to be the note that spawns in random locations after E5 follows you around for the first time.

I've looked at a bunch of guides and forum posts that all say the same thing but I am unable to encounter this final note.

Am I doing something wrong? If not, how can I fix this and find the 4th note?

Honestly, I've never found them all either! Folks on Steam have gained the achievement, so I'm sure it's possible. The game's developer (Josh N) is traveling currently, but hopefully he will answer when he gets back.

I managed to find it by using a 2011 save file of the exact part where Delta-Six can be controlled for the first time after gaining the ability to switch.

It turns out that it was my original save file that was 'corrupt' seeing as I did everything the same way with the new file and I was able to find the last note there.

A friend said it was a rare bug where the last note wouldn't spawn at all.
